Health History and Vaccinations
Please remember to complete your Health History Form (sent in a separate mailing), including documentation of a physical with your primary care provider (mandatory), Medical Provider Request & Consent for Continuity of Medication Management (mandatory), and the Allergy Injection Consent Form (optional) by August 1, 2019. It is imperative that you send in your Health History Form and for the Health Clinic to be informed of Chronic care conditions. I.e. Diabetes. Crohns. Epilepsy etc. Please email these forms to healthservices@mica.edu or fax to 410-225-2416. If you are living on campus, please ensure that we either have verification of your meningitis vaccination or meningitis waiver form. Confirmation of vaccination or completion of the waiver form is required by the State of Maryland. If our office does not have verification of these documents, you will not be allowed to move into your room. For answers to questions about the vaccination requirements, medical care on campus, or the health history form, please call the Office of Student Affairs at 410-225-2422.
Health Insurance
MICA has adopted the following policy that applies to our students; all students are required to maintain health insurance coverage that is in place every day of their higher education career. Please visit MICA's Tuition and Fees page for more information.
Students are automatically enrolled into the MICA sponsored health plan, but they can choose to waive out of or confirm enrollment up to September 15, 2019. Any waivers requested after September 30, 2019 will be assessed a financial penalty. You will receive information in the mail about how to opt out of the insurance plan over the summer. If you are an international student, please review the materials sent to you in a separate mailing about health insurance as the policy has changed from last year. If you have questions about MICA's insurance coverage including steps about opting out, please call the Healthcare Advocate at 1-800-398-8441 ext. 2.
Health Care Services
Student health services for MICA students are provided in the Wellness Center (410-225-4118), located at 1501 West Mount Royal Avenue. Every MICA student pays a mandatory health fee in order to access student health services.
Students in need of emergency care should dial 911 for assistance. If a student has an urgent need that cannot wait until the next business day, he or she may call the student health center at 410-225-4118 for directions on how to contact the Chase Brexton after-hours provider. The student will be connected with an on-call Chase Brexton provider who will assist in identifying the appropriate level of medical attention required.
